What documents do I need to go to the DMV?

When applying for a permit, driver’s license or identification card, the Indiana BMV requires applicants to establish identity, lawful status, residency and proof of Social Security number by submitting original or certified copies of the following documents:

Proof of Identity
• United States birth certificate (citizens born in Puerto Rico must provide a birth certificate issued on or after July 1, 2010)
• Amended birth certificate (must show change of legal name, gender or date of birth or gender and filed with a state Office of Vital Statistics.
• United States passport or passport card
• Foreign passport with U.S. Visa and approved I-94 document
• Consular Report of Birth Abroad issued by the State Department (Form FS-240, Form DS-1350 or Form FS-545)
• Certificate of Naturalization (Form N-550 or Form N-570)
• Certificate of Citizenship (Form N-560 or Form N-561)
• Permanent Resident card issued by the Department of Homeland Security (DHS) or the Citizenship and Immigration Services (Form I-551)
• Employment authorization document issued by DHS (Form I-766)
• Indiana secure identification credential
• Any other document issued by a United States federal agency

Lawful Status
• United States birth certificate or issued by a U.S. territory (filed by the Office of Vital Statistics)
• United States passport or passport card
• Consular Report of Birth Abroad issued by the State Department (Form FS-240, Form DS-1350 or Form FS-545)
• Certificate of Naturalization (Form N-550 or Form N-570)
• Certificate of Citizenship (Form N-560 or Form N-561)
• Permanent Resident card (Form I-551)
• Foreign passport with U.S. Visa and approved I-94 document
• Proof of application for asylum in the United States (Form I-589)
• Notice of Action received by DHS (Form I-797) and accompanied by another document issued by the United States Citizenship and Immigration Service (USCIS)
• Other documents issued by USCIS

Proof of Indiana Residency (Must present two original documents)
• United States Postal Service change of address confirmation
• Indiana voter registration card
• Survey of Indiana property owned by applicant
• Bill from a doctor, hospital, credit card or utility company issued within 60 days of application
• Mortgage, loan contract, lease or rental agreement
• Bank statement or transaction receipted dated within 60 days of application
• Motor vehicle loan payment book (must be current)
• Valid homeowner’s, renter’s or auto insurance policy dated within one year of application
• W-2 form, property tax, excise tax bill or Social Security Administration (SSA) or other retirement or pension benefits statement dated within current or previous year
• Pre-printed paystub dated within 60 days of application
• Indiana Family and Social Services Administration child support check stub, Medicare or Medicaid benefit statement dated within 60 days of application
• Valid and active identification card issued pursuant to the Indiana Attorney General’s address confidentiality program under Indiana Code 5-26.5

Proof of Social Security Number
• Original Social Security card
• W-2 form
• SSA 1099 form
• Non-SSA 1099 form
• Paystub with applicant’s name and Social Security number
• Proof of ineligibility from the SSA with a letter not more than 60 days old

Proof of Name Change
• Valid marriage license
• Decree of divorce
• Court order (must show change of date of birth or legal name)
• Amended birth certificate (must show change of gender)
• Statement from physician (must be signed, dated, include full name and explain gender treatment)
