What documents do I need to go to the DMV?

The Maryland MVA requires applicants to submit proof of age, identity, residency, legal presence and provide a Social Security number or proof of ineligibility. The documents must be an original or certified copy from the following sources:

Proof of Age and Identity
• United States birth certificate filed with the Office of Vital Statistics or equivalent agency (birth documents issued from a hospital, birth registration cards, notifications of birth registration and foreign birth certificates will not be accepted. Puerto Rican birth certificates issued before July 1, 2010 are no longer accepted)
• Valid United States passport
• Consular Report of Birth Abroad issued by the United States Department of State (Form FS-240, DS-1350 or FS-545)
• Valid Permanent Resident Card (Form I-551) issued by the United States Department of Homeland Security (DHS)
• Certificate of Naturalization (Form N-550 or N-570) issued by the United States DHS
• Certificate of Citizenship (Form N-560 of N-561) issued by the United States DHS
• Unexpired employment authorization documents (Form I-766 or I-688B) issued by the United States DHS
• Unexpired foreign passport with valid United States visa accompanied (unexpired or expired) accompanied by the approved I-94 form documenting the applicant’s most recent admittance into the United States

Proof of Social Security Number or Proof of Ineligibility
• Original Social Security card
• W-2 form (not more than 18 months old)
• 1099 form (not more than 18 months old)
• Paystub with applicant’s name and Social Security number (not more than three months old)
• Documentation showing non-work authorized status
• Other proof of ineligibility for a Social Security number acceptable by the MVA

You must change your name with the Social Security Administration (SSA) if there is a different name on your Social Security card.

Proof of Legal Presence (All documents must be verified by the DHS through their Systematic Alien Verification for Entitlements (SAVE) program)
• Valid United States passport
• Consular Report of Birth Abroad issued by the United States Department of State (Form FS-240, DS-1350 or FS-545)
• Valid Permanent Resident Card (Form I-551) issued by the United States Department of Homeland Security (DHS)
• Certificate of Naturalization (Form N-550 or N-570) issued by the United States DHS
• Certificate of Citizenship (Form N-560 of N-561) issued by the United States DHS
• Unexpired employment authorization documents (Form I-766 or I-688B) issued by the United States DHS
• Unexpired foreign passport with valid United States visa accompanied (unexpired or expired) accompanied by the approved I-94 form documenting the applicant’s most recent admittance into the United States

Proof of Maryland Residency (Submit at least two documents from the following sources)
• Vehicle registration card or title with Maryland address
• Utility, telephone, cable or satellite television bill
• Bank statement for Maryland checking or savings account
• Life insurance policy or card (over three years old)
• Property tax receipt or bill
• Proof of home ownership (mortgage account)
• First class mail from local, state or federal government agency (must include both the envelope and contents)
• State or federal income tax return with proof of filing (not more than 18 months old)
• Contract from a financial institution
• Business or sales tax license
• Credit card bill (must be a major credit card)
• Service contract from a residential provider (satellite or cable television, exterminator, gardening or lawn service)
• Maryland voter registration card
• Selective Service Card
