What documents must I fill out at the DMV to get my permit/driver’s license?

All applicants who apply for a driver’s license must show proof of identification by providing one primary and one secondary document or two primary documents as listed below:

Primary Documents
• Driver’s license with photo issued by another state that is not expired more the 31 days
• Arkansas photo identification issued by the Arkansas Department of Finance and Administration, Office of Revenue
• Original or certified copy of U.S. birth certificate
• Certificate of Naturalization
• Certificate of Citizenship
• Resident Alien Card
• Court order that contains the full name, date of birth and court seal
• Military identification with photo
• Military dependent identification with photo
• Valid U.S. passport
• Valid foreign passport along with the required INS documents
• Discharge papers from the U.S. Armed Forces
• Draft registration card

Secondary Documents
• School or employment identification with photo
• Social Security Card
• Vehicle registration or vehicle title
• Marriage license
• Health insurance card
• Medicaid card with photo
• Bureau of Indian Affairs/Indian treaty card (no tribal cards)
• State or federal tax forms (W-2 forms are not acceptable)
• Court order (date of birth not required to be present on document)
• Pilot license
• Concealed handgun license
• Certified school transcript
• Parental affidavit (parent must appear in person and submit notarized documents confirming child’s identity)
• Court records
• Prison release documents
